NOTICE OF PRIVACY PRACTICES Effective December 13, 2023 THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Rayne Medicine Shoppe® ("we", "our", "us") respects your privacy. This Notice explains how we use and share your Protected Health Information ("PHI") and outlines your rights. OUR DUTIES • Maintain Privacy: We protect your PHI and provide this Notice of our duties and practices. • Follow This Notice: We comply with the terms of the current Notice. • Changes: We may update this Notice. The new version will apply to all PHI we keep. You may request a paper copy at any time. HOW WE MAY USE AND DISCLOSE YOUR PHI 1. Treatment: Dispensing medication, coordinating refills, consulting with prescribers, managing your care. 2. Payment: Seeking payment from insurance plans or third parties. 3. Health Care Operations: Quality reviews, staff training, internal audits. 4. Required by Law: When laws or regulations require us to share information. 5. Public Health Activities: Reporting adverse events or preventing disease. 6. Victims of Abuse, Neglect, or Domestic Violence: Reporting to authorized agencies when needed. 7. Health Oversight: Audits, inspections, investigations, or licensure reviews by oversight agencies. 8. Judicial and Administrative Proceedings: In response to a court order or subpoena that meets legal standards. 9. Law Enforcement: Sharing information with law-enforcement officials as permitted by law. 10. Coroners, Medical Examiners, Funeral Directors: Identifying a deceased person or finding a cause of death. 11. Organ and Tissue Donation: Facilitating donation and transplantation. 12. Research: With Institutional Review Board approval or with your written permission when needed. 13. Serious Threats: Preventing a serious and imminent threat to health or safety. 14. Specialized Government Functions: Military, national security, or correctional purposes. 15. Workers’ Compensation: Complying with workers’ compensation laws. 16. Disaster Relief: Assisting authorized agencies in disaster situations. 17. Business Associates: Sharing PHI with service providers who must safeguard it. USES AND DISCLOSURES REQUIRING YOUR AUTHORIZATION We will get your written permission for uses and disclosures not listed above. You may revoke that permission at any time in writing, unless we have already acted on it. YOUR RIGHTS • Request Restrictions: Ask for limits on certain uses or disclosures. While we may not be able to agree to every request, we will consider them. • Confidential Communications: Request contact at a different address or phone number. • Inspect and Copy: View or obtain copies of your PHI. Reasonable fees may apply. • Amend: Ask us to correct information you believe is wrong or incomplete. • Accounting of Disclosures: Receive a list of certain disclosures made in the past six years, excluding treatment, payment, and operations. • Paper Copy: Ask for another paper copy of this Notice, even if you agreed to electronic delivery. COMPLAINTS If you believe your privacy rights were violated, you may file a complaint with our Privacy Officer or with the U.S. Department of Health and Human Services, Office for Civil Rights.
